URGENT CARE SERVICES

• $150 – BASIC URGENT HOUSE-CALL FEE*

• Includes House Call Urgent Care visit with one main complaint that requires minor treatment, includes one in-home rapid test (strep, Mono, flu, urine or glucose). 

• ** COVID TESTING IS SEPARATE COST**

• If fluids added on for sick visit, price discounted to $100 as additional cost (includes nausea or pain medication in fluids)

• After initial 30 min visit,  there will be an additional $50 charged every 30 min thereafter.

VIRTUAL MEDICAL VISIT – TELEHEALTH

• $100 INITAL VISIT AND $75 FOR SUBSEQUENT VISITS

• Telemedicine is an unhurried video or phone call with one of our medical professionals, who will be able to consult and resolve most routine acute non-life-threatening illnesses, refill prescriptions and more.

HYDRATION / IV THERAPY/BOUTIQUE INFUSION

• Includes 1 liter of either Normal Saline or Lactated Ringers and 1 b12 shot done in patients home

• Discounts available for hydration parties, minimum 5 clients for party discount, call for pricing

• Splash is $150

• Revitalize and Immune Boost are $175

ANNUAL CHECK-UP / WELLNESS VISIT WITH LABS

• Includes yearly labs (complete blood count, liver/ kidney/ electrolytes, thyroid, diabetes, cholesterol, and prostate)

AESTHETICS AT HOME​

• Botox: $16 per unit for botox at home

• Dermal fillers for face and lips: $499 per syringe

• Sclerotherapy: $400 for 30 min session

• Pricing based on units or syringes.

• Botox Parties:

  • Minimum 5 injections at party required. If minimum required is not reached, there is a $50 surcharge to host. Cost of product below pending on number of people who get injections​

• Attendees Discount:​

  • 0-5 people: $12 per unit 

  • 5-10 people: $11 per unit

  • 10 people and over: $10 per unit

  • $350 per syringe dermal fillers

  • $250 Sclerotherapy session

• Host discount: discount pending how many procedures done

  • 0-5 people: $10 per unit​

  • 5-10  people: $9 per unit

  • 10 people and over: $8 per unit

  • Sclerotherapy: $200 for 30 min session

CANCELLATION FEE

• $25 if scheduled appointment is missed or cancelled without 24 hours notice

AFTER HOURS COST

• After hour tele-health cost: $25

• After hour home visit cost: $75

OUTSIDE OF SERVICE AREA SURCHARGE (AT HOME VISIT)

• $25 for additional 5 miles outside radius

• $50 for additional 10 miles outside radius

• Call for additional pricing if further past 10 miles

Cash price labs can be done at any Quest or any lab you choose if you want to file with your insurance
